Insurance Information

At Cornerstone Family Medicine we make every effort to provide you with the finest medical care and the most convenient financial options. To accomplish this goal, we work hand-in-hand with you to maximize your insurance reimbursement for covered procedures. If you have any problems or questions, please ask our staff. They are well informed and up-to-date.

At Cornerstone Family Medicine we make every effort to contract with most major medical insurance plans. Please contact your insurance provider to verify if we are in network. If you have further questions, please contact our office.

FAQ

Please contact you pharmacy and ask them to send us an electronic request. These are typically completed within 48-72 hours.

In order for us to release your medical records, you will need to sign a release form. You may drop it off at our office, fax us the form (480)–981-5501 or mail it to us. These will completed within 2 weeks.

We encourage you to schedule an appointment with your provider to review your test results.

Depending on your insurance, you may need to schedule an appointment with your provider before you see a specialist.

Insurance information can change, coverage can change, and it is important to have updated information so claims can be filed appropriately and so the patient doesn’t get stuck with the bill.

Please call our Billing Department, Monday through Friday 9 am to 5 pm at (480) 981-6100.

At each office visit, we ask that patients bring their medical insurance card. If your insurance information, phone number, or street address change, please notify at the time of your visit. It is also helpful to bring a current list of medications you are taking.

7am-3:15pm with a lunch break from 12pm-1pm Monday – Friday we have a phlebotomist on staff to perform all lab draws.

Contact your insurance company. If an error was on the Insurance Carrier side, then have them expedite a payment with a Date. If paid accurately, then file an appeal, your claim may be reviewed and reconsidered

Co-payment is a flat amount due for each visit to a provider.

Deductible is the amount that you must pay before your health plan begins to pay for your bills. Typically, a deductible is a flat dollar amount (i.e. $250.00 or $500.00.) A co-insurance is generally a percentage of covered charges after the deductible is satisfied

We understand that you may have situations demanding schedule change. Please call our office 24 hours in advance to inform us if you need to reschedule or cancel your appointment and we are happy to reschedule or cancel your appointment. A late cancellation fee may apply if you do not call us 24 hours in advance. Your cooperation would help us serve our patients in a timely and efficient manner.

In general, please plan on arriving 15 minutes prior to your scheduled appointment time to ensure we have up to date information and paperwork completed.

We use an electronic appointment reminder system that will send out an email and/or text depending on your contact preference on file.
