Call Now Get StartedDr. Jeff Willden is a board certified, bilingual family practitioner. He has been practicing in Arizona for nearly twenty years. After attending Brigham Young University for his undergraduate degree, he spent a year at Arizona State University doing post-graduate work. After attending medical school at the Kirksville College of Osteopathic Medicine in Missouri, he returned to his home state of Arizona for his residency training years at John C. Lincoln Hospital in Phoenix. He built Cornerstone Family Medicine from the ground up with Dr. Crawford whom he met in medical school.
Dr. Willden has been married for 25 years and has four children. He is a constant learner whose pursuits have included gardening, perfecting his jalapeno jam recipe, fly fishing, fly tying and rod building. He enjoys traveling and spending time with his family.
Dr. Rustin Crawford is a board certified family practitioner who has been caring for patients in Arizona for nearly two decades. He attended Brigham Young University for his undergraduate studies, and went to medical school at Kirksville College of Osteopathic Medicine in Kirksville, Missouri. He came to Arizona for residency training at the John C. Lincoln system in Phoenix. After residency, Dr. Crawford co-founded Cornerstone Family Medicine in 2001.
Dr. Crawford is married and has 6 children. He is active in community and church activities. In his free time, he enjoys reading, fishing, sports of any kind, traveling and other outdoor activities with his family.

Connie McLeod is a Board Certified Family Nurse Practitioner and a member of The American Academy of Nurse Practitioners. Connie graduated, Magna Cum Laude, with a Master’s and Bachelor’s degree in Nursing from Grand Canyon University. She has over 30 years of experience in Nursing with a wide variety of patient populations. Connie has cared for newborns, pediatrics, teenagers, birthing mothers, well men and women examinations, post-operative care, cancer (conventional and natural), and end of life patients.
Her experience has evolved from the hospital, urgent care, and office setting. Connie is an integral part of Cornerstone Family Medicine, providing exceptional care to all patients in the primary care setting. Ms. McLeod was born in Florida, moved to Arizona in 1980, and resides in Gilbert with her family.
David is a bilingual, Board Certified Family Nurse Practitioner. After graduating with an undergraduate degree of Business from University of Phoenix, he attended Grand Canyon University and earned both his undergraduate and post-graduate degrees in Nursing. David is a native Arizonan. He has lived in both Gilbert and Mesa. He served in the United States Air Force (Security Forces) and was honorably discharged after 5 years of service. He was stationed at various locations including Texas, Hawaii, and Korea. He finished his military service at Luke AFB, AZ.
David and his wife Christy have 7 children together: four daughters and three sons. He enjoys being active, going to the gym, and playing all types of sports. In his free time, David loves going to the movies and making memories with his family
Fiona is a Board Certified Family Nurse Practitioner and a member of The American Academy of Nurse Practitioners. Fiona graduated, with a Masters degree in Nursing Leadership before obtaining her Post Masters Nurse Practitioner Certificate from Northern Arizona University. She has over 16 years of nursing experience with a variety of patient populations in the acute care setting including cardiovascular ICU. trauma ICU and flight nursing.
Fiona is originally from London, England and spent the last 15 years in Prescott, AZ prior to moving to San Tan Valley to join Cornerstone Family Medicine in 2018.Fiona is married with 5 children and 3 grandchildren. When she is not at work she enjoys traveling with her husband, spending time with her family and being outdoors with her horses and dogs.
Bianca Garcia Kendall is a third generation native of Phoenix, Arizona. She is a Board Certified Family Nurse Practitioner. She attended Arizona State University where she received her Bachelor of Science in Psychology and Bachelor of Science in Nursing. She returned to school earning her Master of Science in Nursing at Grand Canyon University.
Early in her career, she worked in a variety of clinical settings including preoperative care, post procedure recovery, trauma/ medical intensive care, emergency care, and medical/ surgical units specializing in urology and bariatrics. Bianca is highly committed to the needs of her patients and encourages them to take an active role in decision making that will enhance overall health.
Sara Johansen is a Board Certified Family Nurse Practitioner and a member of the American Academy of Nurse Practitioners. Sara received a Bachelor’s in Nursing from Brigham Young University in 2012. She has since worked in the hospital on medical, surgical, oncology, and orthopedic floors, as well as the ICU. She was the recipient of three Daisy award nominations during her hospital career. Sara graduated magna cum laude with her Master’s degree from Frontier Nursing University. She will continue on to receive her Doctorate in Nursing Practice.
Sara is passionate about helping her patients achieve their goals in health and wellness, as well as managing acute and chronic illnesses across all ages. Sara grew up in California, has lived in Utah, Hawaii, and now Arizona since 2015. In her spare time, she loves running, hiking, skiing, traveling, and spending time with her kids, family, and friends.
Amber Maison is a board certified Physician Associate (PA )graduating in 2018 with a master’s degree in physician assistant studies from A. T. Still University in Mesa, Arizona. She also earned a bachelor’s degree in nutrition from Arizona State University’s downtown Phoenix campus. She has been practicing in family medicine since 2018. Amber has a strong interest in treating mental health, women’s health, and chronic disease prevention and management as well as annual wellness exams and women’s health exams.
Prior to pursuing a career as an Physician assistant, she worked for 18 years’ as an orthodontic assistant. A portion of that time was spent at Barrow’s Cleft and Craniofacial Center at St. Joseph hospital to provide dental care, braces, and orthodontic appliances for children and adults with craniofacial disorders. It is because of these patients that she developed a passion for family medicine due to the strong trust and relationships that came from seeing patients from infancy into adulthood.
Amber Maison was raised in Indiana and moved to Phoenix in 2005 for the beautiful Arizona weather. She lives with her husband, dog, and her in-laws. When she isn’t caring for patients, Amber enjoys reading, completing puzzles, cooking, traveling and spending time with friends and family. She started her own book club with friends and would love your recommendations on the next great book.
Sheena Christiansen is a board certified Family Nurse Practitioner and a member of The American Academy of Nurse Practitioners. Sheena graduated with her master’s degree from University of Phoenix. She has over 15 years of nursing experience from newborns to geriatrics. Sheena has spent most of her career in the urgent care setting. She also has abundant experience working with patients in primary care, women’s health and obstetrics and gynecology. Sheena is passionate about not only treating a patients physical health, but also their mental and spiritual health as well.
Sheena Christiansen was raised in Arizona and currently resides in Chandler with her family. Sheena is married and has 3 beautiful children. She enjoys being active, hiking, paddle boarding, traveling and spending time with her family and friends. Sheena is also very active within her church and loves serving her community.
