How can I renew my prescriptions?
Please contact you pharmacy and ask them to send us an electronic request. These are typically completed within one business day.
How do I obtain a copy of my medical records?
In order for us to release your medical records, you will need to sign a release form. You may drop it off at our office, fax us the form (480)–981-5501 or mail it to us. We will contact you once we receive the form.
A fee may be charged for medical records.
How do I obtain a copy of my test results?
Call us at Mesa Office Phone Number 480-981-6100 or we encourage you to sign up for our patient portal, this allows you access to your results.
How do I request a referral?
Depending on your insurance, you may need to contact us or visit our office before you see a specialist. When we refer you to a specialist we will give you an order and a list of specialist. Once you’ve found the specialist you will be seeing call us so that we may process a referral for your insurance. We will send the referral to the specialist office when complete. This process may take 2-10 business days depending on the insurance.
I gave my insurance information on the first visit, why do I have to share this at every visit?
Insurance information can change, coverage can change, and it is important to have updated information so claims can be filed appropriately and so the patient doesn’t get stuck with the bill.
I have a question about my Billing account. Who do I call?

What am I expected to bring at my visit to the office?
At each office visit, we ask that patients bring their medical insurance card. If your insurance information, phone number, or street address change, please notify at the time of your visit. It is also helpful to bring a current list of medications you are taking.
What are the lab draw hours?
7am-12pm Monday – Friday we have a phlebotomist on staff to perform all lab draws.
What do I do if I disagree with how much my insurance company has paid on my bill?
Contact your insurance company. If an error was on the Insurance Carrier side, then have them expedite a payment with a Date. If paid accurately, then file an appeal, your claim may be reviewed and reconsidered
What is a copayment or a deductible and coinsurance?
Co-payment is a flat amount due for each visit to a provider.
Deductible is the amount that you must pay before your health plan begins to pay for your bills. Typically, a deductible is a flat dollar amount (i.e. $250.00 or $500.00.) A co-insurance is generally a percentage of covered charges after the deductible is satisfied
What is the policy on canceling/rescheduling an appointment?
We understand that you may have situations demanding schedule change. Please call our office 24 hours in advance to inform us if you need to reschedule or cancel your appointment and we are happy to reschedule or cancel your appointment. A late cancellation fee may apply if you do not call us 24 hours in advance. Your cooperation would help us serve our patients in a timely and efficient manner.
Why do I need Preventive Care Visits?
Preventing disease is key to improving our health and keeping rising health costs under control. When we invest in prevention, the benefits are broadly shared. What you need in terms of preventive care depends on your age, your personal risk factors and family history. Depending on where you are in that spectrum, you may be requiring regular preventive care including physicals, routine cholesterol screening, PSA screening if a male, and pap smears and mammograms if you are a female. Routine immunizations are also important. We will encourage you to make your wellness appointments on a regular basis not because our revenue is low, but because your long term health is important to us.
How early should I arrive to my appointment?
In general, please plan on arriving 15 minutes prior to your scheduled appointment time to ensure we have up to date information and paperwork completed.
Will someone call me to remind me about my scheduled appointment?
We use an electronic appointment reminder system that will send out an email and/or text depending on your contact preference on file.
